Mục tiêu: Đánh giá hiệu quả của tẩy giun sớm và bổ sung đa vi chất trên trẻ suy dinh dưỡng (SDD) thấp còi, dân tộc Pakoh và Vân Kiều (Quảng Trị).

Phương pháp: thử nghiệm can thiệp cộng đồng có đối chứng; 284 trẻ bị SDD thấp còi, trong đó 139 trẻ có nhiễm giun, được chia ngẫu nhiên ra 4 nhóm với 4 can thiệp khác nhau. 1) Nhóm Chứng (n=73), không được tẩy giun và uống đa vi chất; 2) nhóm Tẩy giun đơn thuần (TG, n=70), gồm những trẻ có giun, được uống 1 liều Mebendazole 500mg khi bắt đầu can thiệp; 3) nhóm bổ sung đa vi chất (ĐVC, n=72), được uống gói đa vi chất hàng ngày; 4) nhóm kết hợp TG+ĐVC (n=69), vừa được tẩy giun, vừa uống đa vi chất hàng ngày trong 6 tháng.

Kết quả: sau 6 tháng can thiệp, tẩy giun và bổ sung đa vi chất cho trẻ SDD thấp còi có tác dụng tăng hàm lượng vi chất dinh dưỡng (Hb, Retinol, Kẽm) và giảm tỷ lệ thiếu máu, thiếu vitamin A, thiếu kẽm. Bổ sung đa vi chất có hiệu quả tốt hơn tẩy giun đơn thuần. Can thiệp phối hợp bổ sung ĐVC và TG có tác dụng hiệp đồng làm tăng hiệu quả lên tình trạng vi chất dinh dưỡng (VCDD) tốt hơn tẩy giun hoặc bổ sung đa vi chất đơn thuần.

Khuyến nghị: Để phòng thiếu vi chất dinh dưỡng và SDD ở trẻ em, Bộ Y tế, các chương trình dinh dưỡng, cần xem xét ban hành chính sách bổ sung đa vi chất kết hợp tẩy giun cho trẻ từ 12 tháng tuổi ở những vùng nhiễm giun và tỷ lệ SDD cao.

Objective: To evaluate the efficacy of early de-worming and multi-micronutrient supplementation on stunting children who are Pakoh and Van Kieu ethnics in Quang Tri province.

Methods: A cluster randomized, controlled clinical trial was conducted among 284 stunting children, in which had 141 worm infection. The study subjects were divided into 4 groups for different interventions: 1) Control group (CTR, n=73), had no deworming and no micronutrient supplementation; 2) Merely dewormed group (TG, n=70), children with worm infection, received 1 dose of Mebendazole 500 mg at the beginning of the study; 3) Merely micronutrient supplemented group (ĐVC, n=72), had no deworming, received daily micronutrient supplement; 4) Combined deworming and micronutrient supplementing group(TG+ĐVC, n=69), received 1 dose of Mebendazole at the beginning and daily micronutrient supplement in 6 months.

Results: After 6 months of intervention, the deworming and micronutrient supplementation interventions increased micronutrients status (Hb, Retinol, and Zinc) as well as reduced the rate of anemia, vitamin A, and Zinc deficiency. Merely micronutrient supplementation is more efficient than merely deworming. Combined deworming and micronutrient supplementation have more effects on micronutrient situation than merely micronutrient supplementation or merely deworming.

Recommendations: To prevent micronutrients deficiency and malnutrition for children, MOH, the PEM and other nutrition program should consider  the policies to support early micronutrient supplementation combining with deworming for children under 12 months old in the areas with high malnutrition and worm infection.
