Tình trạng Vitamin A (VA) của trẻ 6-36 tháng tuổi sau khi uống VA liều cao và một số yếu tố nguy cơ liên quan được theo dõi trên 233 đối tượng của 2 xã miền núi, tỉnh Thái Nguyên. Kết quả cho thấy tỷ lệ trẻ có nồng độ retinol hyết thanh thấp (<0,7µmol/L), tăng rõ rệt từ 11,6%  sau 3 tháng, đến đến 18,9% sau 5 tháng uống VA. Tương tự, nồng độ retinol giảm  ý nghĩa (p<0,001) từ 1,03±0,42µmol/L xuống 0,84 ±0,39 µmol/L. SDD thể gầy còm và SDD thể nhẹ cân làm tăng  nguy cơ thiếu VA 17,7 lần (p<0,001) và 2,8 lần (p<0,01) so với trẻ không SDD. Trẻ bị tiêu chảy, nhiễm khuẩn hô hấp trong tháng qua, làm tăng nguy cơ thiếu VA 3,42 lần (p<0,001) và 2,14 lần (p<0,05) so với trẻ không bị nhiễm khuẩn. Kiến thức, thực hành phòng chống SDD kém, thực hành sử dụng thực phẩm giàu VA chưa tốt, là những yếu tố nguy cơ của retinol huyết thanh thấp. Chưa thấy tương quan rõ rệt giữa thiếu máu, SDD thấp còi với tình trạng VA. Khuyến nghị: Cần thí điểm bổ sung VA 3 tháng/1 lần cho trẻ ở vùng có tỷ lệ SDD cao >20%; tăng cường truyền thông nâng cao kiến thức và thực hành phòng chống SDD, thiếu VA cho bà mẹ nuôi con và người chăm sóc trẻ.

Vitamin A (VA) status of children aged 6-36 month after the supplementation of high-dose VA, and risk factors were investigated in 233 subjects, in 2 mountainous communes, Thai Nguyen province. Results indicated that prevalence of low serum retinol (<0.7µmol/L) increased significantly from 11.6%  to 18.9% , similarly the serum retinol level reduced from 1.03±0.42 µmol/L to 0.84 ±0.39 µmol/L, respectively at 3 and 5 months after VA supplementation. Wasting and underweight increased risk of VA deficiency to 17.7 times (p<0.001) and 2.8 times (p<0.01); diarrhea and respiratory infection of last month increased risk of VAD to 3.42 times (p<0.001)  and 2.14 times (p<0.05), respectively. Poor maternal knowledge on nutrition and poor practices on eating VA rich foods were risk factors of low serum retinol. Relation between anemia and stunting with VAD was not observed. The VA supplements for children every 3 months in areas with high prevalence of malnutrition, nutritional education improving the knowledge and practice for mother should be implemented.
