Tình trạng dinh dưỡng và một số yếu tố liên quan của phụ nữ có thai khám ngoại trú tại Bệnh viện E năm 2024

Tóm tắt

Mục tiêu: Mô tả tình trạng dinh dưỡng và một số yếu tố liên quan tới tình trạng dinh dưỡng của phụ nữ có thai đến khám tại Bệnh viện E năm 2024.

Phương pháp: Nghiên cứu cắt ngang được tiến hành từ tháng 8/2024 đến tháng 10/2024, phỏng vấn 111 thai phụ đến khám theo bộ câu hỏi thiết kế sẵn và cân, đo nhân trắc.

Kết quả: Trước khi mang thai, tỷ lệ thiếu năng lượng trường diễn là 15,3%, tỷ lệ thừa cân là 12,6%. Kết quả sàng lọc dinh dưỡng có 35,1% thai phụ có nguy cơ suy dinh dưỡng vừa; 7,2% có nguy cơ suy dinh dưỡng nặng. Tỷ lệ tăng cân dưới khuyến nghị là 66,7%, trên khuyến nghị là 12,9%. Các yếu tố liên quan đến suy dinh dưỡng của phụ nữ có thai là tuổi mẹ ³ 35 tuổi (OR=2,9; 95%CI: 1,1-7,3; p<0,05), có bệnh hiện mắc (OR= 6,0; 95%CI: 1,9-18,0; p<0,05), ăn ít hơn/ăn như bình thường trong thai kỳ (OR=4,1; 95%CI: 1,8-9,1; p<0,05). Các yếu tố liên quan tới tăng cân không đúng khuyến nghị: phụ nữ có BMI < 18,5 trước khi có thai (OR=2,1; 95%CI: 0,4-10,0; p<0,05), có nghén (OR=1,7; 95%CI: 0,6-4,5; p<0,05).

Kết luận: Tỷ lệ suy dinh dưỡng và tăng cân không đúng khuyến nghị của phụ nữ có thai còn khá cao. Các yếu tố liên quan đến tăng cân không phù hợp bao gồm phụ nữ trước mang thai có BMI < 18,5 kg/m² và có nôn nghén. Đối với suy dinh dưỡng, các yếu tố bao gồm tuổi mẹ cao, bệnh lý hiện có và chế độ ăn ít trong thời gian mang thai.

Abstract

Aims: To determine the nutritional status and some related factors of pregnant women in the outpatient clinic at the Hospital E in 2024.

Methods: A cross-sectional study was conducted from August 2024 to October 2024, interviewing 111 pregnant women according to a designed questionnaire and measuring anthropometrics.

Results: Before pregnancy, the prevalence of chronic energy deficiency was 15.3%, and the prevalence of overweight was 12.6%. The nutrition screening results showed that 35.1% of the pregnant women were at risk of moderate malnutrition and 7,2% at risk of severe malnutrition. The rate of weight gain below the recommended level was 66.7% and 12.9% above the recommended level. The factors associated with malnutrition in the pregnant women included maternal age ≥ 35 years (OR=2.9; 95%CI: 1.1-7.3; p < 0.05), having a current illness (OR=6.0; 95%CI: 1.9-18.0; p < 0.05), eating less or maintaining usual eating habits during pregnancy (OR=4.1; 95%CI: 1.8-9.1; p < 0.05). The factors associated with inadequate weight gain included women with a BMI < 18.5 before pregnancy (OR=2.1; 95%CI: 0.4-10.0; p < 0.05), and experiencing morning sickness (OR=1.7; 95%CI: 0.6-4.5; p < 0.05).

Conclusion: The prevalence of malnutrition and inappropriate weight gain during pregnancy was still relatively high. The factors associated with inappropriate weight gain included women with a BMI < 18.5 kg/m² before pregnancy and those experiencing morning sickness. The malnutrition was significantly associated with advanced maternal age, pre-existing medical conditions, and inadequate diet during pregnancy.
