Tình trạng vi chất dinh dưỡng và tăng trưởng ở trẻ em Việt Nam

Các vi chất dinh dưỡng (vitamin, khoáng) tham gia vào hầu hết các quá trình chuyển hoá của cơ thể: hoạt động của các enzyme, cơ chất, các quá trình sinh sản và phát triển tế bào. Do vậy hoặc trực tiếp hoặc gián tiếp có vai trò quan trọng với tăng trưởng của cơ thể. Những đối tượng có nguy cơ cao thiếu vi chất dinh dưỡng ở nước ta là trẻ em, phụ nữ tuổi sinh đẻ, đặc biệt là trẻ nhỏ và phụ nữ có thai. Tình trạng vi chất dinh dưỡng có cải thiện trong những năm gần đây, tuy nhiên vẫn ở mức đáng chú ý. Tỷ lệ thiếu vitamin A tiền lâm sàng với mức vitamin A huyết thanh, trong sữa thấp vần ở mức đáng chú ý. Thiếu máu ở trẻ em và phụ nữ có thai vẫn ở mức cao về ý nghĩa sức khoẻ cộng đồng (YNSKCĐ). Thiếu nhiều vi chất kết hợp đồng thời trên trẻ em và phụ nữ. Những tương quan ý nghĩa giữa thiếu vi chất (kẽm, sắt, vitamin A) với phát triển chiều cao, cân nặng cũng đuợc quan sát thấy. Can thiệp bằng bổ sung các vi chất qua đường uống, bổ sung vào thực phẩm... đã cải thiện rõ rệt tốc độ phát triển chiều cao cân nặng của trẻ.Cơ chế điều hoà tăng trưởng của kẽm trong sinh tổng hợp, hoạt tính sinh học của các hormon tăng trưởng (GH, IGF-I) của người và động vật cũng được chứng minh. Thiếu kẽm đã làm giảm sinh tổng hợp, mất tác dụng sinh học của các hormon này và do vậy kìm hãm sự phát triển chiều cao cân nặng của người và động vật. Khẩu phần ăn của các tầng lớp vẫn chưa đảm bảo nhu cầu vi chất dinh dưỡng với trẻ em, phụ nữ có thai, vị thành niên về vitamin A, sắt, kẽm... Việc uống vi chất dự phòng, sử dụng các thực phẩm tăng cường vi chất dinh dưỡng, đa dạng hoá bữa ăn... cho các đối tượng có nguy cơ, là cần thiết nhằm cải thiện tình trạng vi chất và tốc độ phát triển thể lực của người Việt Nam trong những năm tới.

The status of micro-nutrients and growth of children in Vietnam

Abstract:
Micronutrient (vitamin, minerals and trace elements) involved in almost metabolism processes of the body: enzyme activities, reproduction and growth of the cellule. Thus directly or indirectly they have important roles in regulating the growth of the human body. The vulnerable groups of micronutrient deficiencies are children, women in reproductive age, particularly infants and pregnant women. Micronutrient status of Vietnamese population has become better the last decade, however it is still a problem of public health concern. Sub-clinical vitamin A deficiency such as low vitamin A in serum and in breast-milk is still prevalent. Iron deficiency anemia in children and pregnant women is very frequently seen. Multi-micronutrient deficiency is commonly observed in the population, particularly in rural area.
A significant negative association between micronutrient deficiencies (vitamin A, zinc, iron) and weight and height growth was observed. An intervention of providing oral supplementation or foods fortified with the micronutrient ... has improved the growth velocity in weight and height of children. Mechanism of growth regulation activated by zinc in biosynthesis, and biological activities of the growth hormone (GH, IGF-I) were indicated. Zinc deficiency reduces the synthesis, and inhibits the activities of these hormons, and thus inhibiting the growth in weight and in height of the human and animals.
Daily food intake of the population has low levels of micronutrients, particularly the diet of children, pregnant women, and adolescents are poor in iron, zinc and vitamin A. Oral supplementation. promotion of fortified foods, and dietary diversification targeting the at-risk groups are essential interventions to increase micronutrient status and growth velocity of Vietnamese people in the coming years.
