Nutritional status and their risk factors of hemodialys patients in Thanh Nhan hospital Hanoi

Tình trạng dinh dưỡng (TTDD) và một số yếu tố liên quan đã được đánh giá trên 93 bệnh nhân lọc máu chu kỳ tại đơn vị Thận nhân tạo (TNT), bệnh viện Thanh Nhàn trong thời gian 11/2004 - 07/2005. Kết quả cho thấy tỷ lệ thiếu năng lượng trường diễn (CED, BMI <18,5) là 38,7%, trong đó 44,8% ở nam, 28,6% ở nữ. Tỷ lệ bệnh nhân có BMI >23 là 14% (nữ 25,7%, nam 6,9%). Có 47,3% bệnh nhân với mức BMI bình thường (18,5-22,9). Tỷ lệ mỡ cơ thể có xu hướng tăng dần theo tuổi của bệnh nhân, tỷ lệ mỡ ở nữ sinh nhiều hơn có ý nghĩa so với nam giới. Tỷ lệ albumin (Alb) huyết thanh thấp (<35g/L) là 22,6% (nam 24,1%, nữ 22,6%). Có 10,8% số bệnh nhân thiếu kết hợp cả protein và năng lượng. Tỷ lệ thiếu máu là 97,8%; trong đó 25,8% thiếu máu nặng; 43% thiếu máu vừa và 29% thiếu máu nhẹ. Năng lượng khẩu phần trung bình/ngày là 1362 Kcal/ngày, phân bố theo tỷ lệ 17%: 16%: 67% tương ứng cho protid: lipid: glucid. Chỉ số dị hoá protein (PCR) cho thấy 21,5% số bệnh nhân (27,6% ở nam, 11,4% ở nữ) cung cấp không đủ protid cho cơ thể. Protid khẩu phần thấp <1g/kg cân nặng có nguy cơ Alb huyết thanh thấp cao hơn 3 lần (OR= 2,98; p <0,05) so với đủ protid khẩu phần.

Abstract:
Nutritional status and their risk factors have been investigated in 93 hemodialysis patients in Thanhnhan hospiatal - Hanoi, during November 2004 – July 2005. Results indicated that prevalence of chronic energy malnutrition (BMI <18.5) is 38.7% (28.6% in female, 44.8% in male). Prevalence of overweight and obesity (BMI >23) is 14% (25.7% in female, 6.9% in male); 47.3% in normal range of BMI (18.5-22.9). Body fate quantity increased proportional to age of the patient, and significantly higher in female than that of men. Prevalence of low-serum albumin (<35g/L) is 22.6% (24.1% in male, 22.6% in female). Combine both protein and energy deficiency (BMI<18.5 & Alb < 35g/L) was observed in 10.8% of the patients. Prevalence of anemia is 97.8% (25,8% in severe level; 43% in mild levels, and 29% in moderate level). The risk factors related to malnutrition: average of energy consumption of the patient is 1362 Kcal / day with the proportion 17%: 16%: 67% for Protid: Lipid: Glucid respectively. The index PCR indicate 21.5% of the patients (27.6% in male, 11,4% in female) received daily not enough the protein quantity. Low protein quantity consumed (<1g/kg body weight) increased significantly the risk of low serum albumin levels (OR= 2.98; p<0.05).
