Thiếu vitamin A là vấn đề sức khoẻ cộng đồng quan trọng trên thế giới, trong đó có Việt Nam. Nhằm đóng góp thêm cơ sở lý luận và thực hành cho đề xuất thời điểm hợp lý can thiệp bổ sung vitamin A, cải thiện tình trạng vitamin A sữa mẹ, chúng tôi đánh giá hiệu quả bổ sung viên nang vitamin A liều cao tại thời điểm 1 tuần sau sinh tới tình trạng vitamin A trong sữa mẹ trong 6 tuần đầu. Nghiên cứu thử nghiệm can thiệp cộng đồng (đối chứng, mù kép) trên 337 bà mẹ. Kết quả cho thấy, can thiệp vitamin A liều cao 200.000 đơn vị cho bà mẹ tại thời điểm 1 tuần sau sinh có hiệu quả cải thiện tình trạng vitamin A sữa mẹ ở tuần thứ 2 sau sinh (p<0,05); không có hiệu quả ở tuần thứ 4, 6. Bổ sung vitamin A tại thời điểm 1 tuần sau sinh không phải là thời điểm hợp lý để đảm bảo hiệu quả lâu dài cải thiện tình trạng tình trạng vitamin A sữa mẹ. Do vậy, nghiên cứu khuyến nghị cần lựa chọn thời điểm hợp lý hơn để tăng hiệu quả của bổ sung vitamin A liều cao.

Vitamin A deficiency is still a worldwide public health issue in Vietnam. To be able to provide future recommendations on the timing of interventions, and to contribute to the theories and practices of vitamin A supplementation for lactating mothers,  we evaluated the effectiveness of a high dose vitamin A supplementation within the first week after delievery on breask milk retinol status during a 6 week periode. A randomized double-blind community-based trial was conducted on 337 postpartum mothers. The results showed that a high dose vitamin A (200.000 IU) supplementation in the first week after birth  improved breask milk retinol status on the second week after birth (p <0.05); while the supplementation had no effect by the 4th and 6th week. Vitamin A supplementation in the first week after delivery proved not to be the right time to ensure a long-term and efficiency improvement of breast milk retinol status. Therefore, the study recommends that more reasearch is needed to find a more appropriate time to intervene, to increase the effectiveness of high-dose vitamin A supplementation for lactating women.
