Mục đích: Khảo sát tình trạng dinh dưỡng, thiếu máu, nhiễm KST đường ruột, NKHH cấp, tiêu chảy của học sinh tiểu học từ lớp 1-3 (từ 6 đến 9 tuổi) ở một số xã nông thôn nghèo để từ đó có thể đề xuất các biện pháp can thiệp nhằm cải thiện tình trạng dinh dưỡng và sức khỏe cho trẻ em tiểu học ở những vùng nông thôn nghèo.

Đối tượng và phương pháp: Nghiên cứu cắt ngang tiến hành trên 6 trường tiểu học (n=1229) thuộc vùng nông thôn nghèo. Đánh giá tình trạng dinh dưỡng của học sinh theo phân loại của Tổ chức Y tế thế giới (WHO), quần thể tham khảo NCHS. Xét nghiệm Hemoglobin bằng phương pháp HemoCue. Đánh giá thiếu máu dựa vào nồng độ Hemoglobin theo phân loại của WHO. Xét nghiệm phân tìm KST đường ruột bằng phương pháp Kato-Katz.

Kết quả: Tỷ lệ trẻ có CN/T thấp (CN/T< - 2SD) là 30% (mức rất cao), CC/T thấp (CC/T< - 2SD) là 27,5%, (mức trung bình) và CN/CC thấp (CN/CC< - 2SD) là 9%, xếp ở mức cao về YNSKCĐ theo phân loại của WHO. Tỷ lệ thiếu máu (Hemoglobin <11,5 g/dl) là 23,6% xếp ở mức cao về YNSKCĐ. Tỷ lệ trẻ nhiễm từ 1 đến 2 loại KST đường ruột là 54,5%. 39,5% số trẻ mắc NKHH cấp, 8% trẻ mắc tiêu chảy hoặc viêm da tại thời điểm điều tra.

Kết luận: Thiếu dinh dưỡng, thiếu máu, nhiễm KST đường ruột và một số các bệnh nhiễm trùng khác là những vấn đề sức khoẻ của trẻ em tiểu học nhóm tuổi từ 6-9 ở những vùng nông thôn nghèo.

This study was carried out to determine the nutritional status, prevalence of anemia, intestinal parasitic infection, and some common infectious diseases among school-aged children in low socioeconomic areas in rural northern Vietnam.

Methods: A cross-sectional survey was conducted among school children aged 6 to 9 years in 6 primary schools (n=1229), representing the low economic areas of rural Vietnam. Nutritional status of the school children were assessed by WHO classification with the NCHS growth curves as the standard. Capillary blood hemoglobin (Hb) concentration was measured using HemoCue B-Hemoglobin photometer. Anemia is defined using WHO cut-off points. Worm burden was examined from stool using Kato-Katz techniques.

Results: The prevalence of stunting, underweight and wasting was 27.5%, 30%, and 9%, and it is respectively classified as very high, moderate, and high level of public health significant problems according to WHO classification. 23.6% of school children were anemic (Hb<11.5 g/dl), classified as high level of public health significant problem according to WHO classification. 54.5% school children were infected with one or more of intestinal parasites, namely Ascaris, Trichuris, and hookworm. Out of 1,229 school children, 39.5% had upper respiratory infection, 8% had diarrhea or skin infection at the time of the survey.

Conclusion: Malnutrition, anemia, intestinal parasitic infection, and other common illness are major health problems among school children aged from 6-9 years old in the low socioeconomic areas in Vietnam.
